    IBEW Local Union 343 will allow book signing in person as well as allowing members to originally sign books 1 & 2 when terminated, by filling out this form.

  • Book 1 (Required Info)

    If you are terminated, you or the contractor must send us a copy of your termination slip and you will be put back on the book. If you fell off the book and want to get back on, fill out the information below and email it to us.

  • Book 2 (Required Info)

    Must Have Their Home Local Send Us A Copy Of Your Basic Information (email to biz@ibewlocal343.org or fax to 507-282-1562) Including Your, First Middle And Last Name, Social Security Number, Address, City, State, Zip, Date Of Birth, Phone Number (Home And Cell) Email Address, Classification, Along With A Current Travel Letter And A Copy Of A Paid-Up Dues Receipt Along With Your Basic Information Below. We Ask That It Comes From Your Home Local To Keep Your Personal Information Secure.

  • Book signing forms received during normal business hours will be registered by the timestamp of the email. Any forms received after hours will go on at the end of the book on the day we receive it. Signing books by email is temporary and may be terminated at any time.

  • A typed name will be considered your signature. It is the responsibility of the member to make sure we received your information and request to sign the book by email.
